This walkthrough demonstrates how to use Form Lab's EHR integration capabilities with a SMART on FHIR application to complete a BMI form for a test patient as described in the Form Lab Scenario.

Access the EHR System
Open your browser to https://ehr.formlab.fhirlab.net. You'll see the EHR simulator login page:
Log in as "Mr. Representative Pharmacist" by clicking the widget on the right.
Select the Patient
From the patient list, locate and select "Mrs Smart Form" by clicking the widget on the right.
Launch SMART Forms
Click on the blue "Local SmartForms" button on the right of the EHR interface. SMART on FHIR authentication will occur automatically and the CSIRO smart apps application will launch with the selected security context.
You can see the CSIRO forms' user interface in the second screenshot below. You will find the user you selected at the top right, and the patient context (Mrs Smart Form) on the left in the skyblue panel.
Select the Form
Find and open the green BMI form:
- Type
bmiin the search box to filter the list - Select the BMI form with the green icon
Create a new response
Click on the "Create New Response" button on the right
- Set the patient's height to 162 cm
Save your changes
Click either the "Save as Final" button at the bottom right or the "Save Progress" button at the top right to save your changes.
Verify the Data
After saving, the QuestionnaireResponse will be stored on the FHIR server. You can verify this by visiting the URL below, which performs a search for QuestionnaireResponse resources on the FHIR server. By clicking on the blue Read button, you can view the details of the saved response. It should be one of the most recent entries.
